Must be filled out and signed and have a 1x1 headshot of athlete.
Must be filled out by both the parent and doctor who assessed the athlete. Make sure to have a doctor signature along with stamp.
It MUST be a SPORTS physical.
3 Copies of the athletes birth certificate are required. Please make sure to bring the original to sign-ups, as it is needed for verification purposes.
2 Proofs of residency are required in order to enroll your child with any AYF/AYC association. Examples are: Rental agreement
Cable (only if there is a landline attached to your bill)
As a last resource, a Notarized letter of residency.
Every athlete is required to have medical insurance at the time of sign-ups.
The last report card issued to your child is required in order to be able to participate in either football or cheer.
If your child does not have good grades, they will be put on academic probation and progress reports will be required.

All these will require a signature from you.
Equipment Rental Agreement will need to be signed by parent at the time equipment is issued to your child.
There is a $100 fee due at the time of registration. $50 will be refunded once equipment is returned.
There is a $350 fine for unreturned equipment, which will take place via collections.
ALL EQUIPMENT is due back to SCYFC at the end of the last game.
What if we live outside of your area?
-Waivers are available, so yes, you can sign up with us.
Can I volunteer as a coach?
-Yes, if you have experience with either sport, let us know and we will let you know what you need to do.
How old does my kid need to be to play or cheer?
-Kids need to be 5 years old as of July 31st.
-Football players can only play up until 8th grade.
-Cheerleaders can cheer up until they are 18 years old.
